What is Support Coordination?

Support Coordination is a service provided by Achieve Australia that is designed to support participants and families to get the best out of their NDIS plan. It’s about providing tailored options so that you can choose what works best for you, and have control over the type of support you need. We work with you, and your family or carers, to find out the most effective way to use your budget to achieve the goals in your NDIS plan.

What does a Support Coordinator do?

We start by researching and assessing a number of providers to help you choose one that best meets your needs.
We then negotiate with your chosen provider on their services and price, whilst helping you to understand service agreements and bookings arrangements. We can also help to arrange any additional assessments to meet funding requirements.
To help you meet your goals and get the most out of your NDIS plan, we can also provide financial advice and liaise with your Plan Manager, if you have one, on your behalf.
We will help to resolve any NDIS related problems or issues, and support you with having your NDIS plan reviewed in a timely manner.
Using a log system, we take notes on any contact we have with our clients to comply with NDIA standards and to remain transparent.
As an example, we may help you to find new accommodation and coordinate assessments such as Occupational Therapy or Psychology as needed.
We can coordinate assessments if you, or your loved one, needs new Assistive Technology (such as a wheelchair or shower commode).
Once you are ready to go, we help to strengthen and enhance your independence to coordinate your support services moving forward and participate in your local community.
